1. FEES

● No payment is refundable.

2. DISCIPLINARY MATTERS
Refer to the Code of Conduct.

3. UNDERTAKINGS BY THE PERSON RESPONSIBLE FOR THE
PAYMENT(S)
1. To indemnify Smith Horsemanship, Lavender Stables and both their employees
against any injury, harm or other loss caused to any person, horse, or property on
account of the conduct by the student. (Indemnity form at the end of this document
for your completion.)
2. To exempt Smith Horsemanship, Lavender Stables and both their employees from
liabilities incurred on account of any injuries to or illness of the student and/or
horse(s) and agrees and consents that Smith Horsemanship, Lavender Stables and
both their employees may consent to any operation or medical treatment of the
student/horse(s), should such consent be required for medical reasons on an urgent
basis and should it not be possible for the parents of the student to be contacted
immediately.
3. To exempt Smith Horsemanship, Lavender Stables and both their employees from
any liability for loss or damage suffered due to the damage or loss of articles brought
onto the Lavender Stables property.
4. To pay for any damages caused by the student to Smith Horsemanship, Lavender
Stables and both their employees and private property.
5. To notify Roan Smith, 0793581051, or Annatjie Smith, 0823300145, immediately of
any absence of the student from attending sessions at Smith Horsemanship.
6. To make an appointment with Roan Smith, 0793581051, or Annatjie Smith,
0823300145 regarding all matters pertaining to Smith Horsemanship.
7. To accept that Smith Horsemanship has a Biblical foundation.

4. INDEMNITY FORM
Although every precaution will be taken to prevent accidents, neither Smith Horsemanship,
Lavender Stables, the facilitators, all employees, any agents, guests nor other persons
associated with above mentioned parties, will be held responsible for any claims to a
student or horse(s) arising from an accident/incident which may occur whilst on or off the
premises, or at any outing by or on behalf of above mentioned parties or any of the said
persons. This indemnity shall in addition be deemed operative as against any third person
suffering damage consequent upon such injury or death.
1. Remember to give clear instructions to the student regarding medication or any other
special arrangements.
2. The student is not allowed on the premises if he or she has a CONTAGIOUS
ILLNESS.
3. PLEASE inform us if the student will not be attending the session(s).

I, the undersigned parent/guardian/student hereby accept and agree to all the terms and
conditions as stated in this indemnity form.

I hereby also give permission to be photographed and video graphed throughout the training
program and that all material that I am appearing in, can be used by Smith Horsemanship and
Lavender Stables.
